Your player details stay private.

The player information form asks for the parent name, player name, team name, player number, field number, and game time. A player’s first name is enough.

A text you review and send

The website prepares a message on your device and opens your SMS app addressed to Sam at 815-671-7103. You must review the message and tap Send in that app. Opening the app or copying the message does not send it, and this website cannot verify delivery. Coverage is confirmed separately by text.

No website submission or saved draft

This form does not send your details to a website backend, Smartsheet, or analytics service. The details stay in page memory while you fill it out; this site does not save a draft in browser storage or put them into the page address. Refreshing the page clears the form.

Your SMS app and clipboard

Opening the SMS app passes the prepared text to that app, which may keep its own draft. Copy Message puts the text on your device’s clipboard. Messages you send are handled by your messaging provider and received by Sam, who uses the details to review availability and coordinate photographs. No payment information is collected by this form.

No public athlete profiles

Your form details are not displayed publicly. We don’t create searchable athlete profiles or use facial recognition. The portfolio is intended for approved sample images only; gallery captions don’t need to identify an athlete.
